Upon opening the thick leathery journal, a signature would be shown near the bottom middle...

Flipping onto the next page would be little tiny drawings, including medical notes. . .most of which would be lengthy in terms of terminology. The good majority of the first part of the journal would be rather 'boring' to say the least. . . however a few things would be more 'pointed out' than others. . .there would also be a picture on the second page of a 'balding' man with his arms wrapped around a Japanese woman, with him kissing her forehead.

Automatic External Defibrillator (AED)
When a patient is pulseless and apneic they require CRP. However, certain arrhythmias exist such as ventricular fibrillation that need -- Defibrillation. In order to do this, electrodes are attached to the person’s chest and a shock is administered. This shock gives enough electrical stimuli to mimic a Sinus Atrial Rhythm which resets the pacing of the heart and allows Overdrive Suppression of other myocardial foci.
IV Push
There are three parts to the IV:
Lock: The lock is a tubing with two ends - one end has an attachment for a Catheter and the other end is a Lock with prevents fluids from rushing out, but allows fluids to be injected into the tubing. This is important because otherwise when getting a confirmed stick from the vein blood would rush outwards.
Flush: A plastic sterile syringe that contains saline. This is to initially clear the line and flush the veins with fluid. You attach it to the end of the locket and flush the lock until it is filled with the saline, then once the Catheter is in the vein it is flushed again completely injecting all saline into the vein.
Catheter: The needle of the IV push, it is what you insert into the vein. Catheter comes in several sizes 14-26. The lower the # of the Catheter, the larger the needle is.
- Swipe area of IV with alcohol pad (sanitize the area)
- Attach Flush to the Lock
- Flood the Lock by depressing the Flush so the lock is full of saline
- Attach Lock to Catheter
- Apply Tourniquet proximal to the elbow.
- Insert Catheter and follow the AC vein through
- Flood the lock again to inject saline
- Detach the Flush
- Attach medicine via syringe.
Patient Assessment
Form a General Impression
- Is the patient sick?
- How is the patient positioned?
- Is the injury/illness serious?
Assess Level of Consciousness
- Does the pt. respond to your presence?
- Does the pt. respond to verbal/speech?
- Does the pt. respond to painful stimuli?
- Pinch the ear or rub the pt.’s sternum with your knuckle.
- If pt. doesn’t respond to any, there is something very wrong, and condition may be serious. This may require ALS.
Assess Airway
- Head-Tilt Chin-Lift
- Jaw Thrust Maneuver
Assess Breathing
- Do you hear breathing sounds?
- Are they labored? Do you hear rhonchi?
- Is the chest rising and falling?
- When auscultating the lungs (listening to the lungs), do they sound right?
- Administer oxygen:
- Nasal cannula: 6-8 Liters per minute of Hi-flow O2
- Non-Rebreather Mask (NRB): 15 Liters per minute of Hi-Flow O2
- Bag-Valve Mask (BVM): 15 Liters per minute of Hi-Flow O2.
- CPAP (Continuous Positive Airway Pressure)
Assess Circulation
- Is the pt. bleeding?
- Is the pt. cyanotic (blue) in any areas?
- Toes/fingers blue - needs oxygen badly.
- Hands, arms/feet, legs - The patient needs more oxygen IMMEDIATELY
- Groin, neck, lips - the pt. is going to die of hypoxia
Perform Rapid Scan
- Looking for DCAP-BTLS
- Deformities
- Contusions
- Abrasions
- Punctures
- Burns
- Tenderness
- Lacerations
- Swelling
- Head, eyes (pupil dilation), nose, ears, lips, neck (Jugular Vein Distention and Tracheal Deviation), back of neck (Crepitus), chest, abdomen, legs, toes, arms, fingers, then finally the back.
- Is the patient high priority?
- Can the injury/illness be treated on scene?
- What protocol prevents from treatment on scene?
- Cardiopulmonary Resuscitation (CPR)
- Cardiopulmonary Resuscitation, more commonly known as CPR is a medical intervention administered to a person who presents both pulseless and apneic (cardiac arrest). There are several steps to CPR and each one is important.
- Check LoC: A person who is non-responsive may not actually be in cardiac arrest. Attempt to get responsiveness of the patient with AVPU before continuing.
- Check Airway/Breathing: If the patient remains unresponsive you must then assure that their airway is open and uncompromised. If you witness the event leading to their cardiac arrest you may attempt to use the Head Tilt-Chin Lift. Otherwise, use the Jaw-Thrust Maneuver, due to the prospect of compromising C-spine. If they show no signs of breathing (apneic) then move to the next step.
- Check Pulse: You will attempt to palpate a carotid pulse. If the patient is pulseless then you will want to administer an AED immediately if you were present witness to the cardiac arrest and skip to step 5.. Otherwise move to the next step.
- 5 rounds of 30 Compressions with 2 ventilations: These compressions should be administered at 100 per minute. The depth of compressions should be 2 inches. The tempo of compressions is that of the beat to the songs "Another One Bites the Dust", "Disco Inferno", or "Staying Alive".
- Apply AED electrodes to patient's body: There are two electrodes that need to be placed on the patient's body. One goes to the left quadrant of the abdomen and one goes upon the upper right bicep.
- Allow the AED to analyze the myocardial rhythm: AEDs detect heart arrhythmias. Wait for it to analyze the heart's rhythm.
- Shock patient if AED discovers an arrhythmia: If it detects an arrhythmia then it will notify the responder to shock the patient. Ensure that NO ONE is in contact with the body or else they too will receive a high voltage. The last thing you want is to have TWO patients. Be sure to yell, "I AM CLEAR, YOU ARE CLEAR, EVERYONE IS CLEAR!" before shocking.
- Resume chest compressions: Until the patient has return of spontaneous circulation (ROSC) or three or more rounds of chest compressions. If they do not resuscitate by then, you can surely pronounce them dead.
Epinephrine
AKA “adrenaline” excites the flight-or-flight sympathetic nervous system. Heart rate increased, the vascular system constricts, and causes the lungs to need less oxygen to function. Used for cardiac arrest, anaphylaxis, and bleeding.

Tourniquet
A tourniquet is not a last resort. If the wound is inflicted on a limb, you are to apply a tourniquet two to four inches above the wound and joint as the first part of the treatment. Your standard equipment will be a C.A.T (Combat Application Tourniquet.
Once you have applied the tourniquet, you are to get the victim in a controlled environment for repairs. Take in mind, not all fragments will be beneficial when extracted. In multiple cases, sometimes the bullets, pellets, or fragments are left in because removal would cause too much damage. You are not to remove the tourniquet until you are finished with the treatment. A tourniquet is not a last resort. If the wound is inflicted on a limb, you are to apply a tourniquet two to four inches above the wound and joint as the first part of the treatment. Your standard equipment will be a C.A.T (Combat Application Tourniquet.
Treatment is generally remove what you can, clean the wound, repair all damaged blood vessels, clean again, suture the wound.

We just escaped Mercy Hospital . . so many of us died in the process. I believe there was, twenty-four of us. Now only eight... god, Julia I hope you're still alive out there...
Just buried the woman that was with us...the soldier that came with offered his sidearm to one of the other survivors. However when he left, there was some arguments being made, and needless to say, many were injured, and the rest ran away. The man came back with some supplies back from the city, including a calendar. Should help keep track of the dates.

The wildlife seem to be more active now, which is good. Haven't seen any deer but, at least the bugs are making noise. Gets unsettling listening to the sound of silence.

Nicolas helped me find my wedding ring. Was cleaning it and lost it in the mud. Think we looked for a good three to four hours. Now thinking about it, I think it involved with him laughing at me while I basically ate shit.

He confided to me that the reason it took him so long to get back to the group a month ago, was because he was burying his friends. He spent a good majority of the time mourning them in silence before getting said supplies. I was upset with him to say the least but, I have to put it in perspective...if it was any of my family, or god forbid it was Julia...

Nicolas seems to be becoming more and more distant. I'm not sure if there's something bothering him, or if he's deep in thought. I'll see how his mood is in due time, after we head back to the city for a supply run.

In a bit of a small 'fit', which I have no idea where it came from, he said that he was going to go hunting for some "real food", and that he'll be back in a few hours. In the meantime, I got a fire started and going.
Been some time since he left. I'm going to go look for him in the next hour if he doesn't come back.
It's just me now. I don't know where he went. He simply 'vanished'. No gunshots or anything. I guess it's just going to be me now.

Been losing track of time. Had to try and remember what day it was. I'm not always great with dates. . . Julia sort of 'took care' of that. I knew I should have just went back with her and the family. However I couldn't have just left those people back in the hospital. There were many that I saved. What if I wasn't there? Would they have been fine? I guess it doesn't matter now. I noticed that they started moving some people around. I'm afraid to show my face, especially for what 'they' did. I just hope she's safe.

I can't handle this. I don't want to be alone anymore. But I want to live, and I don't know if those things take prisoners. Food and water supplies are low to almost non existent. Something raided the camp, destroying everything in the process. Gathered nearly everything that I could carry and have in my bag. Going to start heading towards New York City. It's a bit of a big place so, might be easier to sneak around.

Came across what I think was supposed to be some form of a bunker. Before taking what was inside, a man burst through a door with something running behind him. He seemed to be a rather, odd to say the least, although rather nice at the same time. I got to meet some others in the group; Callum, Wyatt, Mina, Delilah, Izzy, Ethan, Jacob, and a few others. Don't know who named their little group but, they call themselves "The Long Haulers", and they have a plan to get out of the city; a cargo ship.

We're about to head out. I'm nervous, and scared. I fear that we won't make it, and that we'll die a horrible death. However I need to keep my composure. As a doctor, I have to show others that everything is alright. Hopefully, everything will be alright.

Everything bad that could happen, did. We lost so many people way too quickly.. the odd fellow from the beginning, Delilah, Ethan.. I was lucky enough to patch up Mina earlier in the day. I feel, so very guilty. I left her, Jacob, and Callum behind. I thought they died. I left them all behind for my own safety, and I feel ashamed for it. All I can do if make sure I live long enough for them to prosper... when we got to the ship, a pair of multi-eyed green aliens helped stabilize Wyatt...this gave me time to try and work on Gary but, I was too late. Poor Jacob...he was with me when this happened, trying to keep the poor man alive.. he's an interesting youngster.. full of heart but, brash. I just hope he learns soon and doesn't get himself killed one day...
I later came to save Wyatt's life. To be honest, I was angry at him; he didn't listen to what I said, and to which end I blame him for everything that has happened...but everyone else looks up to him as a leader, and as such I did what needed to be done. In the process of working on him and saving his life, the pair of aliens that were trying to keep him stabilized caused us to go into a plane of existence, if you were to call it that known as 'The Void'. I don't know how long we'll be here but, they say that we may return back to Earth. Or we might even go to other worlds. While this is both exciting, it frightens me at the same time. . .

'It' doesn't like us being here. It even offered me a way back home...however, it said that it wouldn't have been exactly the same, or that things would have been missing. I'm not sure if I understood it correctly but, I couldn't have the chance of entering some, 'portal' to my own 'dimension', and leave Julia behind... I just have to live onto the hope and idea that she is still out there.

We were teleported to a 'new' plane of existence...believe it was some 'borderworld', or connection to what the Vortigaunts call 'Xen'. It looks similar, however it has different structures and 'landscapes.' While here, we entered a facility that was offline, or was offline, until someone made the lights come back on. We were forced to fight with these things called 'Synths', ordered by this 'flying grub-like thing'. In the process, Wyatt was killed.. or at least his physical human body was. I don't know what they did, but they I think, transferred his conscious into a Vortigaunt. I don't know how they managed to do it, but it's kind of hard to question things, considering we were in a world that had 'Oxygen' in it.. it took some time, but he managed to figure out a way to tell 'me' that it was him... poor bastard...

Haven't written an entry in a while, but one of our Vortigaunt friends, 'Khal', was sacrificed to some machine in the void...in this process though, 'Vort-Wyatt', became 'regular-Wyatt'...something of which I felt was off. This couldn't have been him. There was no form of technology involved. Gavroche confided to me that this 'was Wyatt', but at the same time wasn't him.. Alice, one of the new 'crewmates' that was pulled into the void with us figured this out as well. Keeping an eye on him for now.

Figured out the date after landing back home, with the help of Gavroche. We landed just a bit outside the coast-line area of the Baltic Sea. It's cold out here, and the threat of getting shelled by some 'military force' doesn't make things better.

I trained them a bit back on the boat, but I was officially 'assigned' a group; Mina, Tobias, and one other person who I didn't train but was going to; Ellie. I did train Jacob a little bit but after the little stunt back on the Boat, pretty sure no one would want him to help out. I knew I should have trained them on how to administer certain drugs, but I didn't and I nearly payed the price for it. I kind of feel bad for yelling and Jacob and constantly putting him down. I just thought to myself as I was losing consciousness that I would possibly not see Julia again.
Wyatt started to round up everyone's weapons. I gave him my main one, considering I was better suited in just keeping people alive rather than shooting at others. However in the process, he took a gun from a woman named Ellie, from an accidental misfire. At first I thought it was logical, however from the sounds of it, it wasn't her fault. I felt bad for her. It's funny; she has a bit of a temper towards others but, I don't really see it as such...she's in this with us, and everything is going to shit. I'm planning on teaching her and the others more on what to do in certain situations, medically wise anyways.

I was taken in by the military force known as the SRF. I left because Wyatt ordered Jacob and the others to kill a pair of men, in fear of them returning to some 'base' of theirs. I didn't want to stay any longer, in fear of the group either blindly following someone who would kill whomever they 'please'. The SRF aren't exactly any better but, they wish that we have a form of 'equality'. Doesn't even really matter, not anymore that is. I'm sure everyone hates me for leaving, and they should right to do so. I left them behind because I was scared. Scared that I'd be forced to murder someone because of him, or even him ordering my own death. Doesn't matter anymore though...

It's been about, two weeks since I last wrote. Hardly had anytime. Had to keep some poor bastards from dying from their stupidity. This guy thought it'd be a good idea to rush into battle before we had a proper plan. Things went sideways, and fast. All my years of being in the medical field, and I've never seen so much death at once. It's weird to say, but at some point you'd think you'd get used to the smell of death. You never do.

I don't know how Mina would have handled the warfront, as well as Jacob... pretty sure he'd get himself killed. So long as I'm around though, I'll make sure he doesn't go.
After talking to Alice, it seemed that she wasn't 'entirely' upset. I don't think she was 'telling the truth'. To be honest, it unnerved me. I expected to get yelled at by the long haulers, but none of them did. Hell even Ellie was calm with me. I don't deserve this 'passiveness'. I just wish...I don't know... I'll make it up to them in some way, one way or another...
