America, 60% of it, covered in irreversible debris - whether it'd be Xen matter or destroyed cityscapes. The country was torn down day by day after the collapse of its ground forces. Only those pockets of 'resistance' and refugees, were able to have left prior to the outbreak, and Combine's attempt to conquer cities drove people out to farther urban towns. Outside of the major population districts.
Ohio, saw this - in the form of the repurposed University turned refugee sanctuary - people feeling from the outer downtown areas- outwards towards the city's border or just a tad bit away. Northeast or 'N.E' for short was a former Medical University run by the state. Its facilities were able to accommodate medical students domestically or internationally. A place of study for what should've been the world's medical researchers and professional caregivers.
The settlement currently, houses roughly 1,700 people- and has been growing ever since due to their new radio tower. Housing is not the best, but being worked on, for now, the electricity is off but limited to only certain appliances and on at certain times. Food is coming close to only a few months' worth, as the scavenging parties either return empty-handed or don't return at all. The security, of the site, isn't horrible - somewhat good. Installments along the border made it less easy for Xen intruders but still penetrable by raiders
Officially established into a community after members of Ohio's National Guard came to the realization and began understanding the severity of the crisis at hand the University only worked as a forward operating base for them, prior to the efforts made months later. Equipment brought in still remains there - although not much of it. The first order, for the camp, was the establishment of living communes, which was accomplished quite quickly, enough to host several hundred people but all this was thanks to the premade residence of the Unversity. Living quarters were established other things began to fall in, all while roles began to be taken. The commanding officer, the one that was left after the liberation, Col. Garland Carney took charge as the overseer and began to organize the building efforts. Within the first 365 days, they'd only accomplished little, still combatting the new climate and hostilities brought on by the portal storms and other conditions that had appeared the last decade. With only living quarters, a couple of hundred residences, and a food bank set up the progress was going slow but steady.
This slow start didn't hinder them in any way as it created a firm base for them to jump on. As more people came in, upgrades were made, and with the populace rising more hands were free. The settlement's rising population allowed for more ease in the security and food situation. Educated persons developed systems, and experienced people followed along. The idea to return power was brought up and made into a reality, with the solar panels fixed and the wings of certain buildings restored. From that other things sprung up, water catchers, botanical gardens, medical wards, and so forth.
2015, would be with no doubt, the settlement's golden year. A border, 'The gate', had been constructed around the roads surrounding the university. These routes created a sense of security, but also a barrier to keeping entities out. But not all of them, improvements were still to be made. The wall was finished not even thirty days after the new year, and the wall being accomplished led to more hands being free. General work and basic upgrading centered around the settlement's quality of line created a more suitable living environment. Although heating was still not readily available, power and a lack of actual heating devices, a bit of it was restored to the living communes. And days after that - more garden space was made - the 'food source' was expanded for the upcoming harvests. Basic repairs here and there just straightened the facility, and in 2015 - under the oversight of the Settlement's leader Garland 'Blue' Carney.

Power - Not so good
The current power situation could be described as 'stable'. The pre-war solar panels were able to be rebooted, but some of the damage was irreversible.
Food - Deteriorating
Scavenging runs are able to make it back with food, but not enough. The food bank was tapped into several times this year - something not seen in previous years.
Security - Great
Talks of an upgrade to the wall, as well as security teams, are underway. Classes for self-defense and arms training are now mandatory for able-bodied men.
